- ICD-10-CM does not support medical necessity for the CPT
- Two CPT codes unbundle under NCCI PTP without a valid modifier
- Units exceed an MUE without documentation
- Unspecified diagnosis when laterality or type was in the note
Denials software should not promise to “fix”
Credentialing, COB, expired auth, and medical-necessity coverage policy still belong to RCM operations. Marketing that “AI stops all denials” is inaccurate.
