HCPCS Level I vs Level II
Level I is CPT: visits, surgeries, and many professional services. Level II is the CMS set used for ambulances, DME, drugs, and other items CPT does not describe. Mixing the two is a common onboarding mistake for new billers.

- Level II = CMS alphanumeric codes (often A, J, E ranges)

Where HCPCS shows up in medical billing
Outpatient and professional claims use HCPCS/CPT on service lines. Coverage still follows LCDs, NCDs, and payer contracts. Automation can suggest a J-code when the note documents an administered drug; it cannot decide Medicare coverage.
